Two questions - 1. do all the adjusting nuts orient towards the inside of the car? It seems that way so they can be accessed for adjusting. 2. Do people that have these have a problem with them coming out of adjustment? Do you put in a set screw or something to keep them from rotating?

Yes, towards the outside. No set screw.

Thanks - good, towards the outside - tight fit to adjust them? Do you have any issues with them coming out of adjustment?

They should have a locking two piece washer with them that holds them in place once tightened down.
